Scenario 5 is the one that I think would be most difficult to stop. Synder says it should be fairly easy, but I think he overlooks the fact that our media, still, for some reason, takes Trump's statements as fact by default. If Trump says foreign or domestic enemies are to blame, the media will run with that.

Also, Synder overlooks the fact that Trump has spent the last 14 months intentionally dismantling our counter-terrorism operations, making it all that much easier for an event to occur-- or a false flag operation to be detected.
