Could be mistaken but I don't believe you can apply for a medical waiver until after your eligibility is complete. So he can't apply this year. He doesn't have a shot at it unless he has another season ending injury in the next 4 years

You are correct.  This year is just going to count as a redshirt.

Now if he transfers and has to sit out another year, I believe he can apply to gain back another year of eligibility.

No. If he transfers, he would sit due to a decision in his control. Therefore, he would not be eligible for a Five Year Rule Waiver, regardless of medical issues this season. Five Year Rule Waiver applies when you miss two or more years to circumstances outside of your control (e.g., injury).

Now, the NCAA disregards the rules from time to time, but by rule he would not be a 6th year candidate if he simply transferred.
