If a player gets injured early in the season and is granted a medical reshirt but then he transfers schools after the season, does he still need to sit out a year at his new school before he's eligible?

Yes.  Two different redshirt years would be given, and a potential 6th year to use 4 years of eligibility could be granted.

In a transfer, you need to sit out the year "in residence", which is different than the medical redshirt.

Several different things being discussed in this topic.

1) Medical hardship waiver: There is no such thing as a medical redshirt.  A medical hardship waiver essentially doesn't count the year in which someone played a portion of a season as a year of competition.  These waives provide an exception to the 'four year max of competition' rules.

2) The '6th season' MUMac mentions relates to the '5 year clock' (i.e., you have 5 years to exhaust your competition eligibility).

3) Transfers relate to the timing of eligibility to compete at a new school. 

Also.. redshirts are better described as taken, not given.  You're not 'awarded' a redshirt, rather you redshirt by not competing.  A redshirt is in control of the student-athlete and/or institution they are enrolled at.
